Except, that is, for a new smart hotel in Japan . The recently launched ProPilot Park Ryokan, in Hakone, has taken cutting-edge scientific advances to create what must surely be a world-first self-parking slippers. The vital new service the biggest technological breakthrough in hospitality since Henn-na Hotel brought in robots to staff its reception desk allows guests to kick off their slippers without care, safe in the knowledge that the artificially intelligent footwear will tidy itself. Retractable wheels allow the slippers to move around when not being worn, with sensors ensuring they can avoid obstacles while returning to the hotels lobby in a spooky display of independence. Floor cushions and tables at the ryokan (a form of traditional Japanese inn) have also been fitted with automated abilities, allowing guests to rearrange the furniture as they see fit without having to remember where everything went originally. Health Data Breaches on the Rise
Large-scale health data breaches reported by doctors and health plans have been rising steadily, a new report shows.From 2010 to 2013, nearly 1000 large breaches affected more than 29 million individual health records, and more than half resulted from theft or loss of laptops, thumb drives and paper records, according to researchers with access to government data.Hacking incidents more than doubled during those years but still represented less than a third of all breaches.While electronic data security and privacy is not a problem that is unique to healthcare, individually identifiable health data cannot be easily reset or changed once it has been compromised like credit card information can, for example, said lead author Dr. Vincent Liu of the Kaiser Permanente Division of Research in Oakland, California.Electronic health records and other emerging technologies for using health data have great potential to improve the delivery of high-value healthcare, however, we must ensure that our patients' data remains secure, Liu told Reuters Health by email.He and his coauthors analyzed the U.S. Department of Health and Human Services database of breaches of unencrypted health information reported by Health Insurance Portability and Accountability Act (HIPAA) covered entities. They only included breaches affecting at least 500 individuals, and where the information could be traced back to individual patients.Between 2010 and 2013, there were 949 of these large unauthorized acquisitions, accesses, uses or disclosures, involving more than 29 million records.The yearly number of breaches rose from 214 in 2010 to 236 in 2011, 234 in 2012 and 265 in 2013.Most involved electronic health records, and a third involved laptop computers or portable electronic devices.These numbers, published in the Journal of the American Medical Association, only include breaches that were recognized, reported and affected at least 500 records, so they likely underestimate the true number occurring each year.We found that as many as 30 million records were compromised in a four-year span, Liu said. If each of these represented records from a unique patient, it could suggest that as many as 1 of every 11 Americans' healthcare data has been compromised.Hacking increased over the study period, from 12 percent to 27 percent of incidents. But the physical theft of unsecure paper or electronic records accounted for 55 percent of breaches.Thus, while hacking represents a serious threat to the security of healthcare data, improved cybersecurity alone is not a panacea for our data security problems, Liu said.Many breaches occur due to inadequate practices, like failure to encrypt data, or providers loading data onto thumb drives and carrying them around outside of the healthcare setting, said Dr. David Blumenthal of The Commonwealth Fund in New York, who co-authored an editorial about the results.A lot of that is about changing behavior of healthcare institutions and providers, Blumenthal told Reuters Health by phone.Concerned patients can ask at the reception desk of their doctors office or hospital about what security measures are taken with their health information, he said.I dont think that people will necessarily understand what is said to them or that the responses will be accurate, but knowing that people care creates an immensely powerful force for change, Blumenthal said.People may be getting better at recognizing and reporting data leaks, which would explain their apparent increase over time, he said.The breaches all incur federal penalties, but the penalties are not sufficient or stringent enough, he said.Electronic health records do have tremendous value, however, Liu said.Leveraging information through technology, including electronic health records (EHRs) means that doctors in hospitals have access to full health records where and when they need them and . . . have all of the information about all of the patients all of the time, he said.So there are benefits as well as risks that healthcare providers and others will need to continue to address in the new age, Liu said.
Canada Should Be Legally Bound to Its Climate Promises
Passing the 2009 Climate Change Accountability Act could close the gap between our country's ambitions and its actions.During the recent House of Commons emergency climate debate, spurred by an alarming new UN climate change report, there were many politicians talking about the need for Canada to rise to the challenge of climate change. Many of these same politicians acknowledged that, right now, Canada's policies aren't enough to line up with what scientists are saying we need to do.That's around the time that Edmonton-Strathcona MP Linda Duncan rose to speak, calling for "climate change accountability."It might have just been a clever turn of phrase, but it reminded me of the last time I was in the House of Commons watching Question Period. I was there supporting legislation that, if brought back, could close the gap between Canada's climate promises and our climate policy.For me, this story started on Oct. 26, 2009. I was in the House of Commons gallery with around 100 other young people from all across Canada. Most of us had spent the morning meeting with our Members of Parliament, and were hopeful that our presence following the PowerShift Canada conference that weekend, where hundreds of youth from across the country had come to the nation's capital to rally for climate action would spark some action from our elected leaders.It didn't. Question Period droned on as if we weren't there, as if the Copenhagen climate summit wasn't a few weeks away and as if the existential threat of a scorched earth wasn't waiting in the wings.Eventually, a few among us got fed up and started chanting. Most of us joined in, echoing call and response chants about climate justice, stopping tar sands expansion and, most germane to the business in that chamber, calling for the passage of Bill C-311, known as the Climate Change Accountability Act.The bill was remarkably simple. It aimed to enact legislation that would legally bind the Canadian government to meet the targets our country had agreed to in the latest United Nations climate deal at that time, the Kyoto Accord. By law, the prime minister and their cabinet would need to bring legislation to the House to meet our climate targets, and if we weren't meeting them, they would have to bring forward new legislation to change that.In short, it would legally force us to keep our promises exactly the kind of legislation we need now.Earlier this month, the Intergovernmental Panel on Climate Change (IPCC) published the most terrifying text ever assembled about climate change. With input from scientists around the world, the report concluded, in essence, that we have less time to do more when it comes tackling climate change. If we don't, the report explains, the planet will face devastating consequences sooner than anyone previously thought possible.And, it made it crystal clear that we need not only to ensure countries meet the commitments they made under the Paris Agreement, but that we far exceed them.That means Canada's plans, including the recently announced carbon price backstop, are still coming up short, according to a March 2018 report, we're on track to miss our 2020 Paris targets by at least 20 per cent.On top of that, our government keeps backing massive fossil fuel projects, like the TransMountain pipeline, that scientists from the IPCC, from Canada's universities, and some from both keep telling us make it all but impossible to meet those goals.As Catherine McKenna told the House of Commons the day after the emergency debate, "it's easy to have a target, but it's harder to have a plan to do it."The problem is that there's nothing holding our government accountable to their climate promises, but something like the Climate Change Accountability Act could change that. Not only that, it could force future governments to keep the promises of previous one's, helping us to deal with the fact that climate change solutions need to be implemented over decades, while politicians typically think in four-year increments.What's more, back in 2010, the Climate Change Accountability act was supposed to become a law, in part thanks to the support of now Prime Minister/then rookie MP Justin Trudeau.After the dust had settled on the disruption in the House of Commons, sit-ins rolled out all across Canada echoing the demand to pass Bill C-311. Climate activists in cowboy hats occupied Stephen Harper's Calgary office. Toronto residents put on their Sunday best and chained themselves to the reception desk in the late Jim Flaherty's office. All across Canada the protests continued for two weeks as the Copenhagen climate talks began. Near the end of the two weeks, I was thrown out of the House of Commons for the second time, one of six young people who staged a sit-in during a meeting of the House Environment Committee where C-311 was being discussed.Justin Trudeau was on that committee. The next day, he and the other opposition committee members sent Bill C-311 back to the house without amendment.The bill didn't make it to a vote in time for the Copenhagen negotiations, but on May 5, 2010, the Climate Change Accountability Act passed third reading and went to the senate for approval. There, that November, Canada's unelected, Conservative-dominated Senate killed the bill.Shortly after that, Stephen Harper won a majority government and climate legislation in Canada was all but dead for the next five years.Now, nearly a decade later, Justin Trudeau is continuing with Canada's unfortunate legacy of prime ministers that come up short on climate. And, as Catherine McKenna told HuffPost Canada's Althia Raj after the emergency debate, the government has no plans to scale up our climate ambition.In the United Kingdom, the U.K. Climate Change Act requires governments to scale up their climate plans at least every five years following the best available science. We need something similar in Canada, and the good news is that Justin Trudeau and the Liberal Party have already voted for it once.Passing a new Climate Change Accountability Act in Canada could be just what Canada needs. It would give people a tool to help our government raise ambition on climate change and it would help lock in climate policies, like Justin Trudeau's carbon price, from politicians like Andrew Scheer, who don't think we should be doing anything on climate.The recent IPCC report made it clear that we need to do more on climate. That starts with setting a real bottom line that, as a country, we keep our global climate promises. The 2009 Climate Change Accountability Act would have done that. 566 Clients, Two Addresses: How a Theft Investigation Uncovered an Immigration Scam in P.e.i.
It started with a fancy watch nabbed at Halifaxs international airport.But newly released search warrants have revealed the intricate sleuthing that led immigration officials to dive deeply into how hundreds of business immigrants used the same home address in P.E.I.On Monday, charges were laid alleging siblings Ping Zhong and her brother, Yi Zhong, counselled business immigrants seeking permanent residency to provide residential addresses in P.E.I. though they didnt really live there.The 566 clients associated to the two addresses are well beyond what would be expected for a single dwelling residence and motel, investigator Lana Hicks said in one warrant.The nine thick search warrants tell the story of how the probe began on March 30th, 2015, when a border officer red-flagged the watch.Soon, Hicks, an 18-year veteran of the border service, was searching for two witnesses in a potential watch-smuggling case.One man had provided 281 Brackley Point Road in Charlottetown as his address.When Hicks called, she found a hotel reception desk, and decided to run a check through the agencys case management system.It turned out to be the Sherwood Inn and Motel, not far from the airports entrance.A computer search showed 363 immigrants entering Canada under the provinces business immigration program had used the motel as a mailing and/or residential address to federal agencies.Hicks wrote that she decided to open a spin off file to see what was going on.She then found from further database checks that 205 immigrants used 27 Beach Grove Road, the home of hotel owner Ping Zhong, as their mailing or residential address.By now, case number V0184978C was in full swing, as it probed the inner workings of the provincial nominee program, known as the PNP, in which the provincial government recommends potential immigrants to the federal government.The investigators started setting up surveillance teams.They watched the travel plans and lives of various Chinese immigrants under the nominee program, documenting how they claimed the motel as an address while other records indicated they were living in places such as Vancouver or Toronto.Her permanent residency card would have been mailed to (the hotel) and I believe that it would have been received at the hotel and re-mailed to the client in Ontario, Hicks wrote.In September of 2015, the search warrants say a team of three investigators conducted surveillance at the Charlottetown airport as a Chinese woman and her family arrived and were greeted at the airport.They watched as the family went to the Delta hotel downtown, stayed there for a few nights, were taken for their interviews with the P.E.I. Office of Immigration, stopped at the Sherwood for a few minutes, and then flew out of the city.In checks with Canada Post, the investigators say they found mail including key immigration documents being sent to the Sherwood or Ping Zhongs residence wasnt being redirected.The investigators said they observed a person passing mail directly to a Canada Post van outside the hotel, and say they regarded this as suspicious.Reached via cellphone this week, Ping Zhong, 60, declined to comment and said she and her 58-year-old brother have hired a defence lawyer. They will appear in court next month.The Canada Border Services Agency alleges the siblings aided and abetted the misrepresentations by seven immigrants. However, the year-long effort of covert surveillance also claims 566 immigrants in the provincial nominee program submitted the same two addresses as their principal residence to Ottawa between 2008 and 2015.In one search warrant application, urging the judge to allow a search of the Sherwood in February of 2016, Hicks argued that residency fraud hurts the integrity of the Canadian immigration system.Jamie Aiken, the executive director of P.E.I. Office of Immigration, said in an interview Wednesday that hes yet to receive the list of 566 immigrants from CBSA, but that his offices records currently only show 17 cases where the immigrants had the Sherwood motel or Pings home as their residence.He said in the other cases, it appears the business immigrants provided the province with another address on the Island. However, he doesnt know yet how many of those left or ended up dropping out of the program.The claims the CBSA have filed with the courts are concerning, he said. We want to work collaboratively with regulatory bodies with regards to this.We need to continue monitoring this.The arraignment of the Zhongs is set for June 11 at Charlottetown provincial court.The PNP program has long been a topic of debate and controversy in P.E.I.Several Canadian immigration lawyers have argued the program is flawed because it grants permanent residency before theres proof the immigrants have actually stayed on the Island and created a business for a year.Instead, the system nominates would-be immigrants for permanent residency, and the coveted status is awarded after federal vetting for health and security.The immigrants do stand to forfeit a $150,000 escrow deposit, a price many are apparently willing to pay.Last year over half of all the 269 applicants forfeited their deposit and never opened a business, raising $18 million for the small province.

Baseline data will be followed up with national electronic health record data encompassing the entire asylum process and later with national register data among persons who receive residency permits.Ethics and dissemination Ethical approval has been granted by the Coordinating Ethics Committee of the Helsinki and Uusimaa Hospital District. Participation is voluntary and based on written informed consent. Results will be widely disseminated on a national and international level to inform health and welfare policy as well as development of services for asylum seekers. Results of the study will constitute the evidence base for development and implementation of the initial health assessment for asylum seekers on a national level.IntroductionThe peak in asylum applications to Europe between 2015 and 2016 highlighted the need for developing more effective policies for meeting both urgent and long-term healthcare needs of asylum seekers.1 2 An asylum seeker is defined based on their legal status as a person who has applied for international protection and is waiting for a decision on legal asylum, as opposed to a refugee who has already been granted legal asylum.3 Representative population-based information on the health and well-being of asylum seekers is needed to guide decisionmaking and planning of healthcare services for asylum seekers.1 Such information is, however, limited in Europe.Previous studies have focused mainly on one area of health or well-being among asylum seekers, for example, on infectious diseases,46 mental health,7 8 access to healthcare services9 10 and human capital.11 Asylum seekers have been reported to be disproportionally burdened by communicable and non-communicable diseases, including a high prevalence of respiratory, gastrointestinal, dermatological and sexually transmitted diseases,5 6 12 poor dental health5 and physical limitations.10 Prevalence of clinically significant symptoms of depression, anxiety and risk for post-traumatic stress disorder have also been reported to be significantly higher among asylum seekers compared with the general population in receiving countries.7 8 Asylum seekers have also been reported to have a higher prevalence of unmet needs for care, hospital admissions and visits to a psychotherapist but less visits to a general physician compared with the general German population.10Previous studies on the health and well-being of asylum seekers are generally limited in generalisability to the entire asylum seeker population in the given country due to sample size restrictions and sampling methodology. A further common limitation in previous studies is that asylum seekers and refugees are examined as one group. No previous studies among newly arrived asylum seekers including a nationally representative population-based sample and with standardised objective health examination measures have been identified. In Finland, current information on the health and well-being of asylum seekers is limited mainly to infectious disease screening13 and access to healthcare and social services.14 According to a recent healthcare register-based study, the most common reasons for healthcare visits among asylum seekers in Finland were dental and musculoskeletal problems and mental health symptoms.15 More information is available on the health and well-being of persons of refugee origin that have a permanent residence status in Finland. Based on these studies, persons of refugee background have been found to be at a particular disadvantage with respect to a number of health outcomes, including a high incidence of chronic disease risk factors,16 mobility limitations17 and poor mental health.18The main objective of the Asylum Seekers Health and Wellbeing (TERTTU) Survey is to gather systematic and representative interview and health examination data on the health, well-being and health service needs of newly arrived asylum seekers in Finland. Participants in the baseline TERTTU Survey will be followed up using register-based data, providing a unique opportunity to examine trajectories of health of asylum seekers from the very early stages of arrival to Finland. The TERTTU Survey data will be used for identification of key indicators for systematic monitoring of asylum seekers health at a national level. The study will also generate data that can be used for designing health promotion measures in reception centres. Correct and timely identification of vulnerable populations and persons in need of services is likely to have significant impact on the immediate and long-term health of asylum seekers.19 Furthermore, it facilitates optimising of existing resources in healthcare provision.20Methods and analysisThe TERTTU Survey is coordinated by the Equality and Inclusion Unit of the National Institute for Health and Welfare, Finland. The study is conducted in collaboration with the Finnish Immigration Service and the reception centres as a part of an EU Asylum, Migration and Integration Fund project aiming at developing evidence-based health examination protocol for newly arrived asylum seekers.21 The Finnish Immigration Service under the Ministry of Interior is responsible for regulation, organisation, steering, supervision and funding of sheltering and health services for asylum seekers, whereas the reception centres are the main providers of healthcare services.22 Close collaboration with stakeholders provides with a unique opportunity for direct implementation of research findings into practice. Evidence-based development of the health examination protocol will promote equality in the access and quality of services provided for asylum seekers. Stages of the TERTTU Survey are outlined in table 1.View this table:View inlineView popupTable 1 Stages of the TERTTU SurveyA multidisciplinary consortium was formed at the planning stage of the survey consisting of experts from different departments and units of the National Institute for Health and Welfare as well as from other collaborating bodies such as regional authorities, universities, non-governmental organisations and experts working in clinical practice. Conceptual framework of the TERTTU Survey is presented in figure 1. Asylum seekers may arrive at any life stage and their health and well-being are influenced by premigration, migration process-related and postmigration experiences. Sociodemographic and socioeconomic factors mediate the influence of migration-related factors and other exposures throughout the life course.23 Authorities and institutions regulating the asylum process have a central impact on health outcomes of asylum seekers through defining the rights for services and quality criteria for the services provided to asylum seekers.Download figureOpen in new tabDownload powerpointFigure 1 Conceptual framework of the Asylum Seekers Health and Wellbeing (TERTTU) Survey.Data collection of the TERTTU Survey is implemented by eight trained multilingual research nurses. In addition to having a good command of Finnish, languages spoken by research nurses include English, Arabic, Somali, Persian, Dari, Sorani dialect of Kurdish, Urdu, Russian, Portuguese and French. All of the study material given to the participants (including information leaflets and consent forms) have been translated into the most common languages spoken by asylum seekers, which are English, Arabic, Somali, Persian, Sorani dialect of Kurdish and Russian. Material is translated into additional languages over the course of the survey if needed. If the study participant speaks another language than the ones spoken by research nurses, a professional interpreter is used. Professional interpreters are used from accredited companies and are briefed concerning the importance of standardised protocol.Study populationThe study population includes all newly arrived asylum seekers who have applied for asylum for the first time in Finland. Exclusion criteria are persons who: (1) reside in a detention centre; (2) have applied for asylum in another country and are transferred to Finland based on international agreements (internal transfer); (3) have been returned to Finland according to the Dublin regulation; (4) have previously had a residence permit in Finland; (5) andchildren born in Finland. The number of asylum applications varies on a weekly basis. Participants are recruited into the study approximately 2 weeks following registration of their asylum application in Finland. Such sampling frame requires continuous sampling of study participants compared with retrospective sampling that is generally used in health interview and health examination surveys. There are approximately 4060 new asylum applications a week fitting the selection criteria of the study, which enables inclusion of the total population sample into the study.Both adults and children are invited to participate in the study. This enables to link family members. The population-based sample is drawn on a weekly basis from an electronic database containing data on all asylum seekers in Finland and maintained by the Finnish Immigration Services. Each asylum seeker receives a unique identity number that is used throughout the asylum seeking process. Additional registered information includes date of the asylum application, name, sex, date and country of birth, nationality, mother tongue and desired language of the interpreter services.Recruitment of study participantsCurrently (2018), there are altogether 49 reception centres in Finland, out of which 43 are for adults and families and 6are for unaccompanied minors. The number of reception centres varies depending on the number of asylum applications and the speed of the decision processes. A substantial majority of adults and families are initially directed to transit reception centres, where they wait for the asylum interview. Transit reception centres are located in Helsinki (capital of Finland), Turku, Joutseno and Oulu (figure 2). Some newly arrived asylum seekers arrange private housing for themselves. In such case, they are allocated to the geographically closest reception centre for provision of health and social services throughout the asylum process. Unaccompanied minors are usually directed to units for minors, where they remain throughout the entire asylum process. Participants of the TERTTU Survey are recruited at an early stage of the asylum process, therefore permanent study sites have been set up at the transit reception centres. Recruitment is the responsibility of the research nurse. However, reception centre personnel have a central role in facilitating the initial contact between the research nurse and persons belonging to the study sample. An invitation with a set time for a reach-out appointment with the research nurse and a brief information leaflet are delivered to asylum seekers by reception centre personnel. Separate consent is acquired for: (1) the use of the data for research and development purposes within the National Institute for Health and Welfare; (2) sharing anonymised data with researchers outside the institute; (3) record linkage of electronic patient register data to survey data; (3) have been returned to Finland according to the Dublin regulation; (4) have previously had a residence permit in Finland; (5) andchildren born in Finland. The number of asylum applications varies on a weekly basis. Participants are recruited into the study approximately 2 weeks following registration of their asylum application in Finland. Such sampling frame requires continuous sampling of study participants compared with retrospective sampling that is generally used in health interview and health examination surveys. There are approximately 4060 new asylum applications a week fitting the selection criteria of the study, which enables inclusion of the total population sample into the study.Both adults and children are invited to participate in the study. This enables to link family members. The population-based sample is drawn on a weekly basis from an electronic database containing data on all asylum seekers in Finland and maintained by the Finnish Immigration Services. Each asylum seeker receives a unique identity number that is used throughout the asylum seeking process. Additional registered information includes date of the asylum application, name, sex, date and country of birth, nationality, mother tongue and desired language of the interpreter services.Recruitment of study participantsCurrently (2018), there are altogether 49 reception centres in Finland, out of which 43 are for adults and families and 6are for unaccompanied minors. The number of reception centres varies depending on the number of asylum applications and the speed of the decision processes. A substantial majority of adults and families are initially directed to transit reception centres, where they wait for the asylum interview. Transit reception centres are located in Helsinki (capital of Finland), Turku, Joutseno and Oulu (figure 2). Some newly arrived asylum seekers arrange private housing for themselves. In such case, they are allocated to the geographically closest reception centre for provision of health and social services throughout the asylum process. Unaccompanied minors are usually directed to units for minors, where they remain throughout the entire asylum process. Participants of the TERTTU Survey are recruited at an early stage of the asylum process, therefore permanent study sites have been set up at the transit reception centres. Q: I recently hired a young man who does a good job but seems unwilling to address one particular issue. "Joshua" works at our reception desk, answering the phone and assisting customers. He willingly takes on additional tasks and says he loves working here. The only problem is that Joshua has horrible handwriting. Whenever he leaves messages for people, they have to find him and get him to translate. I have repeatedly mentioned that he should improve his penmanship, and other managers have told him this as well. On the rare occasions when Joshua leaves a readable note, I always compliment him. However, this hasn't done any good. I also suggested that he could send messages by email, but he never does. How do I get Joshua to take this seriously? A: Managers often erroneously assume that simply mentioning a concern will magically produce a resolution. Unfortunately, however, that is seldom the case. Performance coaching requires several steps, and awareness is only the first. Given the input from multiple managers, Joshua obviously knows about this problem. And since his notes are occasionally legible, he can apparently fix it if he chooses. So, if awareness and ability are not the issue, that leaves the question of motivation. For Joshua, improved handwriting is simply not important enough to make the effort. And the reason why is actually quite clear. Although you have repeatedly requested change, your words were contradicted by your actions. Continuing to accept his illegible notes sent the message that they were actually OK. To resolve this performance issue, you must reiterate your expectations and then insure that they are met. Therefore, it's time to advise young Joshua that things are about to change. For example: "Joshua, I want you to know that I'm very pleased with your work on the reception desk. However, unreadable notes are still a problem, even though you can write clearly when you try. If you prefer written messages, they will have to be legible. But if they are not, we will have to switch to email." Advise the other managers of this plan, and then be sure to get their feedback. If the mangled messages continue, Joshua will have to lose his writing privileges. Q: Because I am only four-foot-five, people seem to view me as a child. I was recently laid off after 14 years and have begun looking for a new job. However, I feel that interviewers rule me out as soon as they see me. Since I can't grow taller, how do I overcome this obstacle? A: While your short stature is undeniably noticeable, you should not assume that it creates a negative impression. But if childlike assumptions have been a problem, then every other aspect of your demeanour must convey "mature professional adult." This includes dress, hairstyle, speech and introductory handshake, as well as your LinkedIn profile, resume and interview responses. To assess the impact of these changes, solicit feedback from trusted friends or colleagues, especially those who have hiring experience. Because it's been awhile since your last job search, do some practice interviews to increase your confidence.
